I am using Skype with my MBP without a headset and my mother in the UK claims it is better than the telephone.
(A) Check System Preferences > Sound and ensure that the device is selected and the bars pushed to maximum;
(B) if that is no good, look in Utilities where there is an application named Audio MIDI Setup. Check the settings there for selected device.
I am not too sure about Skype settings (mine just works), I am sure that there are settings there for audio as well as video.
